THE SITUATION
Following the passing of his wife, Mr T's family grew concerned about his nutrition, hygiene and growing social withdrawal. He had been managing largely on his own but the situation was deteriorating gradually.
WHAT WE DID
After a home visit and consultation with the family, we developed a care plan for the Personal Care Suite that preserved his daily routines as much as possible. Meals were adapted to his preferences. Staff introduced him to small group activities at a pace he was comfortable with.
OUTCOME
Within three months his appetite improved significantly and his daughter described him as more like himself again. At eleven months, he participates regularly in morning garden walks and has formed genuine friendships with two other residents.

THE SITUATION
Mrs A preferred to remain at home but her children in Singapore were worried about her blood pressure management and the risks of living alone through the night. She was resistant to full residential care.
WHAT WE DID
We started with the Health Monitoring Retainer to establish a baseline and build rapport. After two months, her family and Mrs A agreed to add the Night Watch Service on weeknights. We coordinated with her GP on her antihypertensive regimen throughout.
OUTCOME
Blood pressure readings stabilised over four months. Her GP reduced one of her medications at the six-month review, citing improved management. Her family reported significantly reduced anxiety about the living situation.
NIGHT WATCH SERVICE · 4 MONTHS
A family managing overnight care between two working parents in Puchong
THE SITUATION
The family cared for their elderly mother at home during the day but both parents worked early shifts. The mother had mobility issues and they needed reliable overnight cover five nights a week without the disruption of moving her to a facility.
WHAT WE DID
We assigned two staff members on a rotating schedule to maintain familiarity. The team integrated with the family's existing care approach rather than introducing a separate routine, which helped the mother feel at ease more quickly than expected.
OUTCOME
No falls or incidents over four months of the arrangement. The family reported that the mother slept better with a familiar presence present and that the arrangement removed a significant source of stress for the household.
